Abstract

Heart failure with reduced ejection fraction (HFrEF) is a major cause of hospitalization, and prolonged length of stay (LOS) reflects clinical severity, resource use, and quality of care. This study analyzed factors associated with LOS among patients with HFrEF at Waled Regional Hospital, Cirebon Regency. A retrospective analytical observational study with a cross-sectional approach used medical-record data from January 2024 to December 2025. Total sampling yielded 150 eligible patients. Univariable analysis described patient characteristics; independent t-tests and one-way analysis of variance assessed bivariate associations; and a generalized linear model evaluated independent factors. Mean LOS was 4.24 days (range, 2-9 days). Blood pressure, arrhythmia, valvular heart disease, diabetes mellitus, renal dysfunction, anemia, hyponatremia, chronic obstructive pulmonary disease, and infection were associated with LOS in bivariate analyses (p<0.05), whereas age, sex, coronary artery disease, and potassium status were not. After multivariable adjustment, blood pressure categories, valvular heart disease, and infection remained independently associated with LOS. Early hemodynamic stabilization, prompt recognition of valvular disease, and rapid infection management may help reduce hospitalization duration in patients with HFrEF.
