ANALYSIS OF FACTORS RELATED TO THE EFFICIENCY OF EMERGENCY DEPARTMENT SERVICES BASED ON EMERGENCY DEPARTMENT LENGTH OF STAY (EDLOS) IN NON-TRAUMA PATIENTS AT ROYAL PRIMA HOSPITAL MEDAN

Abstract
Emergency Department Length of Stay (EDLOS) is an important indicator of emergency department service efficiency and patient flow. Prolonged EDLOS may reflect delays in clinical assessment, diagnostic processes, and treatment decisions. This study aimed to analyze factors associated with EDLOS efficiency among non-trauma patients at the Emergency Department of RSU Royal Prima Medan. An analytic observational study with a retrospective approach was conducted using secondary data from medical records of 334 non-trauma patients treated in April 2026. Data were analyzed using univariate, bivariate, and multivariate analyses with binary logistic regression using the enter method. The results showed that 171 patients (51.2%) had efficient EDLOS (≤4 hours), while 163 patients (48.8%) had inefficient EDLOS (>4 hours). Bivariate analysis demonstrated significant associations between arrival time (p=0.049), triage level (p=0.045), diagnostic tests (p=0.032), and Turnaround Time (TAT) (p=0.045) with EDLOS. Age, sex, and disposition status were not significantly associated with EDLOS. Multivariate analysis identified TAT as the only significant factor, with inefficient TAT increasing the odds of inefficient EDLOS by 3.705 times (aOR=3.705; 95% CI: 1.044–13.146; p=0.043). Improving diagnostic TAT should therefore be prioritized to enhance emergency department efficiency and reduce prolonged EDLOS.
